Sažetak
Hypertension frequently occurs in patients with transplant organ like kidney and other solid organ. High blood pressure is a powerful risk factor for cardiovascular events. Immunosuppressive drugs, particularly calcineurin inhibitors have important influence in pathogenesis of hypertension after solid organ transplantation. The aim of the present study was to determine the incidence of hypertension after kidney transplantation. A total of 78 patients (37 women, 41 men) mean age 49.9± ; 12 years were included in the study. The cause of kidney failure was glomerulonephritis in 42 patients, pyelonephritis in 14 patients and polycystic kidney in 10 patients. The spent time on the dialysis before transplantation was 31± ; 30 months. The posttransplant period from 6 to 166 months. The mean body-mass index before kidney transplantation was 23.1± ; 3.5 kg/m2. Hypertension was registred in 14 patients (17, 9%9, before kidney transplantation. During the first posttransplant year hypertension occurred in 47 (79%) patients while in the period of one to three years hypertension occurred in eight (13.5%) and four (6.7%) patients three or more years after transplantation. Mean body-mass index was 24.4± ; 5.6 kg/m2 after transplantation. In all patients with body mass index >25 kg/m2 hypertension was present. Only one drug in treatment of posttransplant hypertension was used in 13 (22%), two drugs were used in 31 (52%) patients and three or more drugs in 15 (25%) of patients. Most often used drugs were calcium channel blockers and ACE inhibitor. Good blood pressure control were obtained in 78% of patients. The study reveals that hypertension in the majority of renal transplant patients develops during the first posttransplant year. Majority patients request therapy with combination two or more drugs.
