Inappropriate use and inadequate dosage of fresh frozen plasma (FFP) is very frequent in spite of precisely defined guidelines. A two-year period (2004 and 2005) retrospective analysis of FFP usage in our institution was undertaken.All patients from departments of surgery and internal medicine were included in the investigation as well asthose from the OR and ICU, while pediatric patients were excluded. Results: In a total of 501 episodes of FFP transfusion 1616 units of FFP were used. In the same period a total of 18 200 units of red cell concentrates were used. At the internal medicine department 57% of FFP was used (303 episodes, 926 units, an average 3.1 units per episode), at the surgical department there were 69 episodes with 222 (14%) units used and an average of 3.2 units per episode, and for the patients in the OR and ICU or 468 (29%) units of FFP were used in 129 episodes with an average of 3.6 units per transfusion episode. Indications for FFP transfusion were: 42 (8%) episodes were caused by peroral anticoagulant drugs reversal, in 362 (72%) episodes of FFP transfusion were indicated based on the results of haemostasis tests (PT< 50%, APTT ratio >1.5, fibrinogen < 1g/L, or other coagulation deficits), acute bleeding with blood usage over 4 units of red cell concentrates (without coagulation tests) were indications for FFP transfusion in 54 (11%) episodes, and there were 43 (9%) inappropriate requests for FFP transfusion. The number of units per episode: there were 8 (2%) episodes with 1 FFP unit, in 203 (40%) episodes 2 FFP units were used, in 249 (50%) episodes 3-5 FFP units, in 36 (7%) episodes 6-10 units of FFP, and in 5 (1%) transfusion episodes over 10 units of FFP were used. Considering the overall FFP usage, especially in relation to red cell concentrates use, it could be affirmed that the FFP usage in our institution is low. Analyzing the FFP indications and dosage in the studied period we may be satisfied with the results, which are evidently the consequence of good clinical transfusion practice in our institution and laboratory testing of haemostasis at our Institute for transfusion medicine.
