Sažetak
Background. Placental site trophoblastic tumor (PSTT) was first described in the uterus. PSTT of the uterus can be fatal, since 10-15% of patients have metastases and/or recurrence in spite of chemotherapy. So far only one case of “ pure” PSTT in a male genital tract has been described, in a 16-month-old boy, who was cured with orchidectomy. A case of a 24-year-old man with PSTT in a late recurrence of a nonseminomatous germ cell tumor of the testis was also recently described, with metastases to regional retroperitoneal lymph nodes. Case presentation. We present a case of 24-year-old man who had a right testicular tumor, discovered due to macroscopic enlargement of the right testicle. Serum levels of beta-hCG were elevated, while alpha-fetoprotein levels were normal. Patohistological examination after radical orchidectomy revealed a tumor consisting of teratoma (65%) and PSTT (35%). PSTT component was human placental lactogen (HPL), cytokeratin and inhibin positive, while beta-hCG and alpha-fetoprotein were negative. Due to high Ki-67 index (more than 90% in PSTT component) laparoscopic retroperitoneal lymphadenectomy was performed and it has revealed 15 lymph nodes without tumor. Conclusion. Presented case is the third case of PSTT in a male genital tract described so far in the literature. Despite high proliferation index, tumor had benign clinical behaviour, and three years after orchidectomy patient is alive and well, without any additional treatment.
