Sažetak
Aim: The aim of our study was to establish the efficacy of resective neurosurgical treatment in patients with pharmacoresistant epilepsy. Methods: In the group of operated patients (14 patients) mean age was 33 years and monthly number of seizures was 2-10. Twelve patients had complex partial seizures and two patients simple partial seizures, 9 of them with secondary generalisation. Three patients had febrile convulsions in childhood and 2 patients birth trauma. Control group consisted of 10 non-operated patients, mean age 38 years, with monthly number of seizures 3-10. Seven patients had complex partial seizures and 3 patients simple partial seizures, 9 of them with secondary generalisation. Three patients had febrile convulsions in childhood and 4 patients birth trauma. In both groups patients were treated with 2-3 antiepileptics. After the operation pathohistological findings revealed hippocampal sclerosis in 5 patients, temporal ganglioglioma in 3 patients, hippocampal oligoastrocytoma in one patient, temporal dysembrioplastic neuroepithelial tumour in one patient, frontoorbital pylocitic astrocytoma in one patient, temporal epidermoid cyst in one patient and petroclival meningioma in one patient. Postoperative radiation was performed in two patients (one with oligoastrocytoma and another with dysembrioplastic neuroepithelial tumour). Results and conclusion: After operation remission was accomplished in 10 patients, while 4 patients still had 1-2 seizures per month, due to recurrence of brain tumour. In the group of non-operated patients number of seizures remained the same. We can conclude that surgical resection together with antiepileptics was a good treatment choice in our group of patients with pharmacoresistant epilepsy.
