Sažetak
Aim: To determine the incidence of trisomy 3 and presence of oncoprotein BCL10 in patients with Hp gastritis, MALT-type lymphoma and diffuse large B-cell lymphoma (DLBCL). Importance of these abnormalities on the course and outcome of the disease, their relation and possible influence on the transformation of MALT-type lymphoma into DLBCL were evaluated. Patients and methods: Twenty Hp gastritis patients, 20 MALT lymphoma patients and 20 patients with DLBCL were selected for the study. The main pathomorphological feature of the first group was excessive immunological reaction with the lymphoid nodules or follicles formation (LN/LF) in H. pylori + gastric mucosa. The second group was characterized by the MALT-type lymphoma and 14 of these patients were Hp+. The third group consisted of 20 DLBCL of the stomach, 11 of them H. pylori +. H. pylori and BCL10 protein were evaluated by the histological and immunohistological methods. Trisomy 3 was detected using FISH on formalin fixed paraffin embedded tissue sections. Results: Intranuclear expression of BCL10 protein in twelve MALT lymphoma patients and in two DLBCL patients was found. There was no trisomy 3 in the first group. In seven patients with MALT lymphoma and three patients with DLBCL trisomy 3 was found. Conclusion: Severity of H. pylori infection does not correlate with the intensity of inflammation, intranuclear expression of BCL10 protein or trisomy 3. Intranuclear expression of BCL10 in some DLBCL patients supports possibility of the MALT-type lymphoma transformation. Finding trisomy 3 more frequently in MALT-lymphoma group of patients (35% vs. 15%) suggests that a part of DLBCL transform from MALT-lymphoma, however part of them arise de novo. Trisomy 3 and intranuclear expression of BCL10 protein did not show statistical significance as predictors of patients' survival.
