Sažetak
Aim: By Maastricht guidelines concerning the age limits, investigate endoscopically detected gastric carcinomas and lymphomas, as well as gastric and duodenal ulcers in two groups of patients, aged under 45 and above 45 years, i.e. among younger and older patients. Methods: 1323 endoscopically examined patients were divided in two age groups: younger than 45 (N=398 ; 30%) ; F/M 157(39%)/241(61%) and older than 46 years (N=925 ; 70%) F/M 408 (44%)/517(56%). In patients with gastric carcinoma (GC) and lymphoma (GL), gastric ulcer (GU) and duodenal ulcer (DU) clinical symptoms were classified as following: no symptoms (NS), epigastric pain (EP), weight loosing (WL) and previous bleding (PB). Results: Younger N (%) NS (%) E P (%) WL (%) PB (%) GC 8 ( 2) 2 (25) 5 (63) 3 (38) - GL 5 (1.3) 2 (40) 2 (40) 2 (40) - GU 48 (12) 3 ( 6) 39 (81) 11 (23) 8 (17) DU 219 (55) 26 (12) 183 (84) 33 (15) 43 (20) Older: GC 70 (7.6) 11 (16) 40 (57) 36 (51) 15 (21) GL 14 (1.5) 5 (36) 6 (42) 2 (14) 1 ( 7) GU 280 (30) 79 (28) 180 (64) 33 (12) 63 (23) DU 315 (34) 55 (18) 235 (75) 53 (17) 64 (20) Conclusion: The incidence of GC is lower among the young (p< 0.05), but 8 cases of GC in not negligeable. GL is in equal proportions in both groups. GU is more frequent in older and DU in younger, as it was expected. More than half of younger patients with GC, GL and GU have no alarm symptoms and according to Maastricht s agreement would be in danger to miss endoscopy and undergo only eradication therapy (H.pylori posit:ive: GC 87.5% ; GL 80% ; GU 82%, DU 95%).
