Sažetak
Background: The identification of H.pylori as a major causal agent in carcinogenesis will enable a more complete understanding of the role of infection organisms in gastric neoplasm's.
Aims: To evaluate a commercial available complement fixation test (CFT) (Institute Virion, Ruschlikon, Zurich, Switzerland) and ELISA (Helori-test, Trieste, Italy) to detect H.pylori infection in clinical and histological groups of gastric cancer (GC) and non Hodgkin lymphoma (NHL) patients.
Methods: 57 patients with clinical and histologically confirmed GC and 17 NHL were studied. H.pylori infection status was assessed by determining IgG antibodies to this organism using CFT and ELISA assay commercial kits (previously validated in endoscoped patients with sensitivity and specifity of CFT were 92 and 80% respectively, and those for ELISA were 98 and 93%) compared to results of CLO-test and Giemsa stained of gastric biopsy specimens.
Results: The overall seroprevalence of H.pylori infection in GC patients was 44/57 (77%) by CFT, and 52/57 (91%) by ELISA assay; in NHL patients 10/17 (59%) by CFT, and 12/17 (71%) by ELISA assay. The table shows the seroprevalence of H.pylori infection in GC and NHL patients according to the Clo-test and Giemsa stained gastric biopsy specimens.
Diagnosis CLO/Histol. pozit. CLO/Histol. negat.
CFT + ELISA + CFT + ELISA +
GC 29/33 (88%) 30/33 (91%) 15/24 (63%) 22/24 (91%)
NHL 7/10 (70%) 9/10 (90%) 3/7 (43%) 3/7 (43%)
Conclusions: Prevalence of H.pylori infection was higher using ELISA than CFT in GC and NHL patients whose CLO-test and histology were positive, and was also higher in GC patients whose CLO-test and histology were negative. ELISA has higher sensitivity than CFT, so it better shows a previous H.pylori infection and its association with GC. Seroprevalence of infection, using ELISA and CFT, was 43% in NHL patients whose CLO-test and histology were negative (no association of H.pylori infection with NHL).
