Sažetak
Burkus et al. forgot to mention the main difference in the surgical procedures between these two studies. When metallic interbody cages, i.e., LT– CAGE devices (Medtronic Sofamor Danek, Memphis, TN) were used rh-BMP-2– soaked sponge was placed in the hollow central portion of the cage and no additional sponges were placed outside the devices.1 On the other side, the rhBMP-2– soaked sponge was placed into the central portion of the bone dowel with an additional rhBMP-2– soaked sponge placed between the bone dowels. The additional rhBMP-2– soaked sponge between the bone dowels was in direct contact with stem cells from vertebral endplates which are known as main targets of BMPs. As those conditions are more favorable from those inside the metallic cages or allograft dowels, the additional rhBMP-2 sponge is a very probable reason of reported different healing pattern, i.e., new bone formation firstly outside of the bone dowels.
