Background: Elevated erythrocyte sedimentation rate (ESR) is a common finding in patients with chronic renal failure (CRF), both in those treated conservatively and in those on dialysis therapy. Therefore, the value of ESR determination is questionable. Elevated ESR is generally considered to be the sequel of the chronic renal failure itself rather than of the mode of the treatment used. Methods: ESR and a number of laboratory parameters were determined in a group of 42 hemodialyzed patients on three occasions in two-month intervals. Patients with malignant diseases, history of acute infection of whatever localization in the month preceding the study elevated body temperature, postoperative state and increased transaminases were excluded. Results: On initial measurement, 'normal' (≤ 24 mm/3.6 ks) and elevated ESR were found in 16.67% and 83.33% of patients, respectively. χ 2-test yielded similar statistically non-significant ratios also on the repeated measurements (14.29% vs. 85.71 and 16.67% vs. 83.33%) (χ 2(1, 2) = 0.07, p > 0.05 ; χ 2(2, 3) = 0.07, p > 0.05 ; χ 2(1, 3) = 0.00, p > 0.05). The mean ESR values were elevated on all the three measurements. t-test for repeated measurements showed no statistically significant changes with time (ESR1 = 53.81 ± 30.08 mm/3.6 ks, ESR2 = 57.26 ± 30.93 mm/3.6 ks, and ESR3 = 56.05 ± 35.34 mm/3.6 ks ; t(1, 2) = 1.23, t(1, 3) = 0.62, and t(2, 3) = 0.33, p > 0.05 all). There was a statistically significant correlation of ESR with leukocytes mean erythrocyte corpuscular volume (MCV), total protein, and α 2- and γ -globulins, statistically non-significant positive correlation with fibrinogen and c-reactive protein (CRP), and statistically non-significant negative correlation with hematocrit and albumin. Conclusions: ESR is significantly elevated in most but not in all patients on hemodialysis. In absence of some additional reasons, ESR undergoes no substantial changes with time. ESR values are predominantly influenced by plasma factors. ESR determination in the stable phase is of low diagnostic value.
