Sažetak
Objective: To establish current recommendations regarding prevention strategies for a first stroke. Introduction: Although treatment of acute stroke has the potential of reducing death and disability, it is likely that prevention will more effectively reduce the ravages of stroke. Considerable evidence in the literature supports an active approach towards the primary prevention of stroke. It is reassuring that much of this evidence comes from randomised trials and systematic reviews that have considered risk factors and have identified clinically reasonable treatments and alternatives. Furthermore, many of these treatments (appropriate use of anticoagulants, aspirin and antihypertensives) are also cost effective. Evidence: A literature review was carried out using the Meedline&reg ; data-base search for 1990 through 2001. Guidelines, meta-analyses, statements, and overviews on prevention of a stroke were reviewed. Conclusions: Six conditions (hypertension, myocardial infarction, atrial fibrillation, diabetes mellitus, asymptomatic carotid artery stenosis and elevated blood lipids) and four life-style factors (cigarette smoking, alcohol use, physical activity and diet) were identified as well-documented risk factors for stroke. Less well-documented or potentially modifiable risk factors were those with either less clear epidemiological evidence or without evidence from randomized trials demonstrating a reduction of stroke risk with modification. High-risk or stroke-prone individuals can be identified and targeted for specific interventions. Several interventions that modify well-documented and treatable cardiovascular and cerebrovascular risk factors can reduce the risk of a first stoke.
