Sažetak
Background and Purpose: The aim of this study was to investigate the effect of topically administered lidocaine and bupivacaine on plasma cholinesterase (PChE) activity in children. Materials ad Methods: After elective inguinal hernia repair, we administered topical local anesthetics to provide effective postoperative analgesia. In 10 children 0.5 % lidocaine and in 10 children 0.25 bupivacaine were administered in sufficient volume to fill the operative wound, prior to the wound closing. The control group included 12 children in which postoperative analgesia was achieved by some other method. The age of the children was between 6 and 10 years. PChE activity was determined 30 minutes after administration of anesthetics. Results: Administration of bupivacaine did not inhibit PChE activity, while the lidocaine administration significantly inhibited PChE activity (p<0.01). Conclusion: Inhibition of PChE activity with lidocaine can compromise the metabolization of other drugs which are matabolized by PChE, especially when they are applied simultaneously. Preoperative determination of PChE activity by screening for PChE "poor metabolizers" could help us choose drugs that are not metabolized by PChE to avoid their prolonged activity.
