The prostate cancer, as one of the most common cancers in men is a subject of numerous investigations. P504S is a gene recently found to show elevated expression in prostate cancer and precursor lesions, but not in normal prostate tissue. A product of this gene is alfa-Methylacyl-CoA recemase (AMACR), a mitochondrial and peroxisomal enzyme involved in the beta-oxidation of branched-chain fatty acid. It can serve as a molecular marker of prostate cancer with high sensitivity, but lower specificity because it is over-expressed in number of adenocarcinomas, notably colorectal carcinoma as well as in hepatocellular, renal cell and urothelial carcinomas. In normal tissue high expression of AMACR was identified in liver, kidney, salivary gland, lung and gallbladder. Fine-needle aspiration of prostate as a means of diagnosis of prostate carcinoma is underestimated, despite its advantages. We perform this qualitative study, with the aim to determine immunocytochemical expression of AMACR on cytologic specimens and its value for cytological diagnosis of prostatic lesions. Data are gathered continuously from January to June 2006. Smears obtained by fine needle aspiration of prostate are examined both by cytological analysis (MGG stain) and by immunocytochemistry (Dako EnVision system and primary antibody P504S). Expression of YMYCR is compared against cytological diagnosis, PSA level and where possible with histological diagnosis and other clinical parameters. AMACR is potentially an important tumor marker and may be of value in detecting premalignant lesions and early carcinomas of prostate since diffferentiating them poses the greatest challenge in cytology.
