Aim: Salivary gland tumours constitute a heterogeneous group and they are a challenge for cytologist to make an accurate diagnosis. The aim of this study was to see the value of ultrasound guided fine needle aspiration cytology (FNAC) in differentiation cytomorhologically similar tumours of parotid gland. Material and methods: Within five years 267 ultrasonographically guided FNAC of salivary gland lesions have been performed. We acquired adequate material from 264 (98.9%) patients. The smears were stained with Papenheim and Papanicoalou staining and cytochemistry. The cytological diagnoses were compared with histological ones. Results: A total of 155 patients underwent surgery out of which 77 patients with tumour mixtus, 24 with adenoid cystic carcinoma and 6 with carcinoma in pleomorphic adenoma. These three entities are sometimes morphologically very similar and therefore they are a challenge for cytologist to make the accurate diagnosis. The ultrasound could be very helpful in such cytological doubtful cases enabling cytological accuracy of 99%. Conclusion: Close co-operation of ultrasonographist and cytologist enables accurate preoperative cytological diagnoses of salivary gland tumours and therefore should be considered in the first line investigations of these lesions
