Objective: Salivary gland tumours constitute a heterogeneous group and they are a challenge for cytologist to make an accurate preoperative diagnosis. The aim of this study was to see the value of ultrasound guided fine needle aspiration cytology (FNAC) in differentiation tumours of parotid glands, especially cytomorphologically similar tumours: pleomorphic adenoma, ca in pleomorphic adenoma and adenoid cystic carcinoma. Materials and methods: Within ten years 410 ultrasonographically guided FNAC of salivary gland lesions have been performed. We acquired adequate material from 405 patients (98.8%). FNAC was performed using 23G needle attached to 20ml syringe. The smears were stained with Pappenheim, Papanicoalou and cytochemical staining (PAS, blue alcian… ). The cytological diagnoses were compared with histopathological ones. Results: A total of 274 patients underwent surgery out of which 185 (67.5%) with benign tumours and 89 (32.5%) with malignant tumours. The most patients were operated because of pleomorphic adenoma – 96 patients. A total of 34 patients were operated with adenoid cystic carcinoma and 7 patients with carcinoma in pleomorphic adenoma. These three entities are sometimes morphologically very similar. Therefore they are a challenge for cytologist to make the accurate preoperative diagnosis because the surgical treatment is different. Comparison between ultrasound and cytology shows a tendency of grouping certain cytodiagnoses towards some ultrasound images enabling accurate diagnosis in doubtful cases. Sonography allows targeting clinically undetectable lesions suggesting abnormalities while cytology gives diagnosis. Ultrasound guided FNAC enables specificity of 99.5%, sensitivity of 98.9% and diagnostic accuracy of 99, 3%. Conclusion: Close co-operation of ultrasonographist performing ultrasound and cytologist performing FNAC enables accurate preoperative cytological diagnoses of parotid gland tumours, especially similar ones. Therefore this method should be considered in the first line investigations of these lesions
