Sažetak
This review shows the devlopment of the Croatian Model of diabetes health care and its inclusion in the St Vincent Declaration, with the aim of decreasing the morbiditiy and mortality of diabetes and its complications. The Model is organized at primary, secondary and tertiary health care levels. After the administrative changes from 1993, specialized health care for diabetic patients is delivered through County and Regional Diabetes Centers, and the Diabetes Reference Center (Vuk Vrhovac Institute for Diabetes, Endocrinology and Metabolic Diseases), a scientific and teaching institution and WHO Collaborating Center. The article covers the epidemiology of diabetes in Croatia and organizational aspects and evaluation of diabetes health care. The prevalence of diabetes in Croatia is 2.35%. There is an average of 57 diabetic patients per primary health care team. The Croatian Model of diabetes health care can be adapted to different settings and can serve as a model for other noncommunicable diseases.
