Sažetak
Recent studies have demonstrated an association between ketoacidosis and diabetic neuropathy. The aim of this study was to determine whether repeated episodes of ketoacidosis could predict pathologic cardiac rhythm detectable by conventional electrocardiography. Twelve-lead electrocardiograms obtained from 45 persons with IDDM were examined for tachycardia, bradycardia and arrhythmia. The patients were asked about their hospitalization for ketoacidosis. The following variables were stepwise entered into a multiple logistic regression model: age, diabetes duration, number of ketoacidoses, HbA1c, serum lipids and smoking status. The results indicated some patients to be more prone to ketoacidotic episodes, women more frequently than men. Data analysis showed that age, diabetes duration and serum lipids were independent predictors of cardiac arrhythmia. The number of ketoacidoses experienced by each patient was not a significant predictor of arrhythmia, indicating that atherosclerotic changes might have been involved in the pathogenic mechanism. The effect of repeated ketoacidoses on cardiac autonomic neuropathy needs to be assessed by the use of autonomic nervous system tests.
