Sažetak
The aim of this work was to evaluate prognostic value of : cytomorphologycal features of thyroid papillary carcinoma, iltrasonographic(US) features of thyroid gland nodules and lymph nodes of the neck region, for appearance of thyroid gland nodules and lymph nodes of the neck region, for appearance of intraglandular and paraglandular carcinoma dissemination. The study included patients with cytological diagnosis of papillary carcinoma in the period from 1st of June 2002 to 31st of May 2003. There were 100 patients and all of them underwent surgical tretment, total thyroidectomy with paraglandular dissection. Following cytomorphological parameters were anallyzed: cell's polymorphism, multinicleation, intranuclear inclusions, psammoma bodies, follicular formations, Hurthle like cells, presence of connective tissue. We also analyzed US features of the nodules in thyroid gland on wich FNAB was performed: position, echo structire, margins of a nodule, size and presence of calcifications. Intraglandular and paraglandular dissemination were determined postoperatively by patohystological examination of resected tissue. Conclusion : There is no statistically significant relation between morphological and US characteristics of papillary carcinoma and his behaviour. The size of nodule is not important for dissemination potential. Total thyroidectomy with partial resection of lymph nodes( depending on extent) must be performed in every patient with papillary carcinoma.
