Sažetak
This analysis was performed to evaluate presentation, therapy and outcome of the thyroid carcinoma (TC) in children and adolescent in comparison to adult patients (pts). We analysed 1041 patients treated for TC in our department. Pts were divided in 4 groups in view of age: 1.Children in pre-puberty period: 12 pts younger than 12 years (yrs) of age, 2. Teenagers /adolescent group: 34 pts between 12-21 yrs of age, 3.Young and middle aged group: 578 pts between 22-50 yrs of age and 4.Eldrly group: 417 pts between 51-80 yrs of age. Mean follow up in the groups was 11, 13, 7 and 5 yrs, respectively. Male/female ratio was 1:1, 4 ; 1:4, 6 ; 1:5 ; and 1:4, 4 ; respectively. Total thyroidectomy was performed in all children and adolescent, and in 98% of adult pts. Lymph node dissection was performed in pts with LN metastases (LNM). Radioiodine ablation was performed in 82% of children and adolescent, and 92% of adult pts. Pts with 131I positive distant metastases (DM) were treated with additional 131I therapies. In these four groups papillary carcinoma was found in:100%, 85%, 82 % and 72% of pts, follicular carcinoma in 0%, 3%, 12% and 14% of pts, Hürthle cell carcinoma in 0%, 0%, 2% and 2%, medullary carcinoma in 0%, 12%, 4%, and 10% of pts, respectively, and anaplastic carcinoma in 1% of the forth group, only. Extrathyroid tumour infiltration was found in 50%, 8%, 7% and 15%, multicentric foci in 58%, 30%, 24% and 25%, LNM in 75%, 44%, 26% and 23% of pts, and DM in 25%, 9%, 5% and 9% of pts, respectively. Two pts from the first group died (16%) during the period of follow-up, nobody died from the second group, 1% died from the third group and 6% died from the fourth group. Of the pts treated in our Institution 4% were children and adolescent. They had more advanced disease at the time of initial diagnosis when compared to adult pts, especially when pts younger than 12 yrs of age are compared to young and middle aged, who had the best outcome of disease.
