Sažetak
Although the first permanent pacemaker was implanted in Croatia in 1964, the Croatian Pacing Registry was established in 1993, when Republic of Croatia has become a sovereign and independent country. So, this report is the firts detailed analysis of cardiac pacing in Croatia. Data from 1997 were collected from ten pacemaker centers, concerning 774 new pacemker implantations and 187 replacements. Regarding to all Croatian population, it means 161 new implantation per milin inhabitants. Of the ten implanting centers, the majority use less than 90 pacemakers per year, and only two of them can perform prepacing electorphysiological study. The mean age of patients at first implantation was 70.8 years. The atrioventricular block (57%), chronic atrial fibrillation with bradycardia (20%) and sick sinus synrdome (17%) were the leading causes for permanent cardiac pacing. Hypertrophic cardiomyopathy has become an indication in 1% of patients. VVI pacing was using in 5o%, VVIR pacing in 27%, dual chamber pacing in 17%, VDD pacing in 5% and atrial pacing in 1% of patients. VVI/VVIR pacing was still the most common mode of stimualtion in atrioventricular blcok (77%) and sick sinus syndrome (63%). The cardioverter defibrillator was implanted in 3 patients only. The number of pacemaker implatation has increased in Croatia 1997. However, the use and the selection of the pacing mode are still inappropriate in majority of the centers, and the implantation rate of cardioverter defibrillators is low. Data of this report are the straight consequence of two major problems in Croatia today: post-war economic recovery and health cure system in transitional country.
