Sažetak
On year before her diagnostic examination, a 51-year-old women accidentally became aware of the hearing impairment in her left ear, while she was phoning. She noticed that hearing impairment progressed. Otoscopic findings were normal. Tonal audiogram showed normal hearing in the right ear, and hearing impairment in the left ear with the response from 80, 75 dB. The examination of the vestibular nerve indicated a gradual paresis of the left nerve, which was compensated. Tympanometry indicated a neural hearing impairment of the left. In the neurological status, beside hearing impairment, there can be a deviation of the tongue to the right, a moderate corneal reflex, and instbility in Romberg, with falling down to the right. Audio potentials, BAEP, was done. Acoustically evoked potentials with normallatencies and amplitudes was registered on the right, acoustically evoked potentials with prolonged latencies of the I and II waves were registered on the left, while the latencies from pyramid of the temporal bone has to be made with special reference on the width of auditory meatus of the inner ear on the left and pontocerebellar angle. The CT of the pyramids of temporal bones in the fenestra for the bone with high resolution showe that the pneumatization of pyramis was regular from both sides and cellulae are free, with preserved septa. Inner auditory meatus is dilated on te left to the width of 1 cm. Structures of the inner ear on the left corespond to normal. The findings of the pyramid of the temporal bone on the right are completely normal. In findings of the pyramid of the temporal bone on te right are completely normal. IN fenestra for endocranium are slight hyperdensity in the left pontocerebellar angle can be noticed. After givin i.v. contrast densities grow up to 57 HU. The change is 34x28 mm in size. The formationspues the IV ventriculus slightly over medial line, and it is surrounded only slightly with oedematous halo of cortex. Everything indicates that it is neurinoma statoacusticus in question. The importance of application of BAEP in the differential diagnostics of impaired hearing can be seen froom the above mentioned. The advantages of this techique are the simplicity of application, without dangerous radiation, and the possibility of repetition of the examination at different times, which enables us to monitor the course of the disease, its progression and after getting examination findings we can also use other sophisticated tests.
