Sažetak
Recent research shows that Helicobacter pylori infection is leading to oxidative stress in gastric mucosa. Despite a successful microorganism's eradication, manifestations of oxidative stress in remain at least few months. Therefore there is evidence of need of supplemental remedies to improve prooxidant/antioxidant balance after antimicrobial treatment. The aim was to study the intensity and distribution of 4-hydroxy-2-nonenal (HNE) immunostaining in the gastric mucosa in duodenal peptic ulcer (DPU) patients and healthy volunteers (HV) and to evaluate efficiency of oil derived from Amaranthus cruentus seeds (AmO) used in addition to standard treatment (ST). Methods 36 PUD patients and 26 HV were enrolled. Patients with DPU were divided into two groups: group 1 (n=18) received ST according to Maastricht Consensus II (2000). The other group (n=18) after ST received 1 ml of concentrated AmO for 1 month. Control group consisted of HV who did not receive any treatment and 4-5 weeks after Hp eradication. HNE was examined after immunohistochemical staining on gastric mucosa samples. Certain levels of HNE in the cytoplasm of glandular cells were determined in all studies subjects. In 23.3% of DPU patients HNE accumulation was also determined in nuclei what indicates more profound oxidative stress with possible destruction of macromolecules, including nucleic acids. Interestingly, none of the HV group subjects was found to have nuclear HNE accumulation. Importantly, no positive changes in HNE in gastric mucosa were observed in group 1 after treatment. It was demonstrated that HNE level lowered in group 2, especially in corpus of the stomach. Such results highlight the importance of supplementary treatments modalities which can efficiently improve aerobic metabolism in DPU patients. DPU patients were demonstrated to have signed of oxidative stress by the HNE accumulation in gastric mucosa. Inclusion of AmO into management plan of DPU was found to be efficient because of its ability to reduce the signs of oxidative stress. Further research is needed to implement this new approach in treatment and prevention of DPU.
