Introduction: Measuring of total HDL and LDL cholesterol and triglyceride is not enough to determine one’ s complete lipid status, so this test panel expanded by measuring apolipoproteins ; mainly Apo B and Apo A-I. Many studies reported that Apo B/Apo A-I ratio is better predicting factor for developing cardiovascular events than LDL cholesterol. Aim:To investigate whether a high Apo B/Apo A-I ratio is significantly associated with the risk of developing stroke. Methods: We evaluated 70 patients with ischemic stroke and 68 age and sex matched controls without cerebrovascular disorders. Apo A-I and Apo B were measured by immunoturbidimetric method (Olympus). ApoA-I concentration has shown normal distribution and results were compared using student t-test, while apoB did not show normal distribution and Mann-Whitney test was used for Apo B and Apo B/Apo A-I ratio. Results: Mean concentration of Apo A-I in stroke patients was 1.33+/-0.29 g/L and in control group 1.41+/-0.24 g/L. The difference was not statistically significant (P=0.072). Median concentration of Apo B in stroke patients was 1.02 9/L and in control group 0.95 g/L. The difference was not statistically significant (P=0.21). Statistically significant difference was found in Apo B/Apo A-I ratio, 0.82 (0.64-0.93) and 0.67 (0.57-0.86), for stroke and control group, respectively (p=0.0189). Conclusion: Our results indicate that Apo A-I and Apo B concentrations were not different between stroke and control group, while Apo B/Apo A-I ratio was significantly higher in stroke group, suggesting that Apo B/Apo A-I ratio could ba a potential new risk factor for ischemic stroke.
