Clozapine possesses a relatively affinity for the D4 receptors. It has been reported that low dose of clozapine improves dyskinesias in chronic Parkinsonian patients. The aim of this study was to evaluate the efficacy and safety of clozapine in treatment of levodopa induced dyskinesias. 30 patients with idiopathic PD and dysabling dyskinesias (F/M 17/13, age range 41-72 years, the duration of disease 5-15 years, and the duration of levodopa treatment 4-15 years) we included in the double blind, placebo controlled crossover study with clozapine vrs placebo. PD disability was ela luated from motor part UPDRS complemented with dystonia score. In addition rigidity quantification was performrd using computerised dewice for the elbow joint. The evaluation was performed during levodopa test. Clozapine treated PD patiend showed clinically and statistically significant improvement of dyskinesias already one week after therapy was induced. PD motor disability score and/or rigidity measurment do not change significantly. Placebo treated patients did not show any clinically significant change from base line at any time point. This is the first double blind study that indicates that low dose clozapine may be potential benefit in management of disabling dystonias in PD.
