Sažetak
We present the case of a middle age female with a declining heart failure, who died in a cardiogenic shock despite vigorous diuretic, cardiotonic, and vasodilator therapy. A history of diabetes mellitus and electrocardiographic finding of myocardial ischemia suggested a possible but unconfirmed ischemic etiology of the underlying disease. Unfortunately, endomyocardial biopsy, crucial for the exact diagnosis, and consequently proper therapy, was not feasible. The autopsy revealed the giant cell myocarditis. We conclude that a progressive intractable heart failure, in the absence of a clear etiological factor, must arouse clinical suspicion of a giant cell myocarditis.
