Sažetak
Background: Transcranial Doppler (TCD) can be used as a confirmatory test in brain death. There are certain factors that limit its usage, but noninvasiveness and bedside evaluation may be a privilege in highly unstable patients. The aim was to present the feasibility of TCD in brain death confirmation. Patients and methods: Thirty five patients with severe brain lesions leading to brain death were treated and observed in Neurological and Neurosurgical Intensive Care Unit during 3-years period. The appropriate test was chosen according to patient condition, appreciating the restrictions in accordance with the test protocol. The main restriction for usage of TCD was craniotomy or scull defects as a result of trauma. Severe hypothermia, hemodynamic or cardiac instability were the main limitation for the radiological tests requiring transportation. Some patients underwent repeated testing with the same or different methods. Results: There were 26 patients with neurotrauma, 7 aneurismal subarachnoidal hemorrhages, 1 arteriovenous parenchymal hemorrhage, 9 hypertensive parenchymal hemorrhages, 2 ischemic strokes. As a primary test TCD was used in 24 patients, brain scintigraphy in 2, multislice CT angiography (MSCTAG) in 8, cerebral angiography in 1 patient, and the diagnose was confirmed in 19, 3, 7 and 1 patient, respectively. Five patients, in whom TCD was applied, died during the observational period, and in two hemodynamic spectra were inconclusive. Thirty-three out of 35 patients developed the complete instability syndrome of hypothermia, hypotension and diabetes insipidus. Conclusion: TCD is the most feasible test for brain death confirmation.
