Sažetak
The Na/H exchanger is a membrane protein that is expressed in virtually all mamalian cells. Genetically determined stimulation of this protein in salt-sensitive hypertension and type 1 diabetes with nephropathy may be responsible for the generation of hypertension either by elevating intracellular Na- and consequently Ca- concentrations and thereby increasing vascular smooth muscle tone or by stimulating proliferation in the sence of primary cardiovascular hypertrophy. The same may hold for chronic salt loading in which case the exchanger may be stimulated by a mild systemic acidosis that develops from increasing renal tubular HCO3-loss following the suppression of angiotensin II.
