Sažetak
Recent knowlege about carcinoides , the localisation, their possible intermediary forms and direction of growth have changed therapy. Kulchitsky cells, the basis of this type of tumor, can change clinical appearence. We retrospectively analyzed 99 patients - which represents 2.4% of all operated lung neoplasmas in Croatia during the observed period. The sample is 52 (52.52%) female and 47 (47.47%) male patients, with average age of 43.08 years. The most frequent symptoms are: recidivant pneumonias, in 46, hemoptyses in 29, without symptoms 12, pneumonias with hemoptyses in 10 patients. Preoperative tumors were verified in 75 patients. In 55 patients lobectomies was performed, bilobectomies in 14, bronchoplastic procedures in 13, segment resections in 12 and in 5 patients pulmectomias. In all patients system mediastinal lymphadenectomy was done. The average size of the tumors was 26mm. PHA showed 83 typical carcinoides, and out of which 10 has metastases into lymphatic nodes, while 16 atypical carcinoides out of which 7 had metastases into regional lymphatic nodes. Local invasion into the parenchima was found in 44 patients. Ten years of survival was established in 82% patients with typical carcinoides, and five years survival in atypical in 57% patients. We would recommend bronchoplastic procedures in typical carcinoides, resection in all atypical and in those typical carcinoideswhere the damages distal of tumor are irreversibile or in those carcinoides which infiltrate transbronchal into parenchyma, with obligatory lymphadenectomy.
