Oncogenic Human papillomaviruses (HPV) have been associated with malignant transformation of the epithelium and mucosa. It is a major risk factor of oral hyperplastic lesions development and further malignant transformation of some of these lesions. The purpose of this study was to examine the occurrence of HPV infection and the prevalence of different HPV genotypes in oral lesions. The study included 120 subjects (45 men and 75 women, mean age 46.8 years) with different lesions of oral cavity: 42 leukoplakia, 22 squamous papilloma, 15 lichen planus, 10 hyperplasia, 5 verruca vulgaris and 26 oral inflammatory lesions as controls. Oral specimens were collected by cytobrush from 9 different topographic sites in the oral cavity. HPV DNA was detected by polymerase chain reaction (PCR) using two sets of universal primers (MY09/MY11 and L1C1/L1C2) and type-specific primers for HPV 6/11, 16, 18, 31 and 33 as previously described (Grce M et al., Anticancer Res 21:579 ; 2001). Out of 120 specimens, 28 (23.3%) were HPV positive containing either single (19.1%) or multiple (4.2%) HPV types. Besides 12 (10%) cases of undetermined HPV types, the most frequently observed type was HPV 16 (9.2%) followed by HPV 6/11 (6.7%), HPV 31 (5.8%), HPV 18 (4.2%) and HPV 33 (0.8%). HPV DNA was identified in 40% verruca, 31.8% papilloma, 30% hyperplasia, 20% lichen planus, 19.2% oral inflammatory lesion and 19% leukoplakia. Oncogenic HPV types 16, 18 and 31 were predominantly associated with leukoplakia, lichen planus and papilloma, while HPV 6/11 with oral squamous papilloma and especially verruca vulgaris. Undetermined HPV types were equally distributed in various oral lesions, except in epithelial hyperplasia identified in each case and in oral verruca vulgaris not found in any case. Multiple HPV infections were common in verruca vulgaris and inflammatory lesions found in more than half positive cases. Our results support the notion that the infection with HPV could be responsible for at least some oral hyperplastic lesion and these lesions should be monitored closely to avoid malignant outcome.
