Sažetak
Renal biopsies of 36 children with isolated microhematuria of proven glomerular origin were examined by light(LM), immunofluorescent(IF)and electronmicroscopy(EM).On LM 17 patients had normal glomeruli, 4 had focal segmental glomerulosclerosis, 13 mesangial proliferative glomerulonephritis(GN) one had membranoproliferative GN and one focal GN.IF revealed 3 cases of IgA nephropathy, in the others IF was negative.EM showed diffuse GBM attenuation in 10 patients with normal glomeruli and in 6 of them subsequently the diagnosis of benign familial hematuria was established.In 17 patients(47, 7%)EM revealed GBM alterations consistent with Alports disease(7 of them had normal glomeruli on LM), and 4 of them developed clinical signs of Alports syndrome somewhat later.The patient with membranoproliferative GN on LM, was proved to have DDD by EM. Our results point to the necessity of performing renal biopsy in children with isolated microhematuria which is by many authors considered to be a minor abnormality where glomerular pathologic changes are not likely to be found.We conclude that in such patients renal biopsy is justified and that it should be always analyzed by LM, IF and EM as well.
