Sažetak
Objective: To assess the value of cytologic diagnosis based on the ovarian cyst aspirate analysis.
Study design: cystic fluid was analysed in 207 patients with 211 non-neoplastic and neoplastic lesions of the ovary. In 30 cases, ultrasound-guided aspiration biopsy was preoperatively performed, whereas in 181 cases aspirates were obtined during the procedure of laparascopy or laparatomy. Histologic diagnosis was used as a gold standard in the assesment of the diagnostic value of cytology.
Results: According to the cytological criteria of the specimen's adequacy there were 127 adequate samples, 80 inadequate samples and 4 unsuitable samples. Histologic analysis revealed 48 tumor like lesions, 122 benign tumours, 19 tumours of borderline malignancy, and 18 malignant tumours. On classifying the cytologic findings into benign and malignant ones, the specificity (Sp) and negative predictive value (PV-) of the cytologic diagnosis for 207 preoperatively and postoperatively obtained ovarian cysts aspirates was 94% and sensitivity (Se) and positive predictive values (PV+) 70%. When the group of borderline tumourswas included in the cytologic classificiation, the values of Sp and PV-, and Se and PV+ were lower, i.e. 93% and 66%, respectively.
Conclusion: Aspiration cytology can be used as one of the parameters in differential diagnosis of tumour like lesions nad cystic tumours of the ovary
