Sažetak
Evaluation of mild cognitive impairment (MCI) is important because it could enable early medical management in order to prevent severe dementia of Alzheimer's or vascular type. In our pilot study, patients admitted to University Department of Neurology due to first ever stroke/TIA (PG, N=81, mean age 56&#177; 7, 0 years) and controls without signs of cerebrovascular disease (CVD) but with vascular risk factors present (CG, N=45, mean age 53&#177; 6, 0 years) were tested using Mini Mental State Exam (MMSE) and Montreal Cognitive Assessment (MoCA) on admittion and retested after three and after six months. In all subjects standard brain computed tomography (CT), Color Dopler Flow Imaging (CDFI) of carotid arteries with intima-media thickness (IMT) measurement and transcranial dopler sonography (TCD) were performed. In 69, 1% (56) patients multiple risk factors were present. Control subjects most often had arterial hypertension (AH), hyperlipoproteinaemia and diabetes. All subjects had normal initial MMSE, in 57, 2%(52) patients and 9, 6% (23) controls MoCA score was decreased (< 26 points). Six months later, mean MoCA score in PG was 18, 87, mean MMSE was 25, 25. Patients with stroke and multiple risk factors showed statistically significant cognitive decline compared to patients with TIA. The greatest cognitive decline among CG after six months was assessed in those with AH and increased IMT. In subjects with cerebrovascular risk factors early signs of cognitive impairment exist even before the CV disease becomes clinically evident. MoCA proved to be appropriate tool for cognitive evaluation of patients with mild cognitive impairment of vascular type.
