Sažetak
Introduction: Inverted papilloma (IP) is benign tumor of nose and paranasal sinuses. Squamous cell carcinoma (SCC) appears synchronously or metachronously to IP. Authors report 3-32%of IPs to be associated with SCC. It's obvious that identification potential markers for malignant alteration of IP would be of great importance.p53 is well-known tumor suppressor gene. Its product have important role in cell cycle control and apoptosis. Mutations of p53 gene are documented in nearly 50% of all human cancers. Ki67 is one of the most prominent proliferation markers showing influence on biologic behavior or being associated with other prognostic factors in different tumors. Material and Methods: We analyzed 58 specimen of IP including 8 cases of synchronous and 2 of metachronous SCC. Patients had diagnosed IP in period from 1982-2001 at Departments of Pathology in Zagreb , Osijek and Budapest . Immunohistochemistry was performed on formalin fixed, paraffin embedded tissue, using DAKO antibodies and LSAB+ staining kit. PC based image analyzer (VAMSTECH, Zagreb ) was used to determine the rate of nuclear staining. Statistical analysis was performed. Results: p53:16 specimen stained positive on immunohistochemistry. Among all cases of SCC arising in IP, except one as well as one of two recurrences of another carcinoma. IP preceding SCC stained weakly positive in multiple areas, contrasting negative staining of overt IP. Fisher’ s exact test showed prognostic significance for malignant transformation (p=0.008).It supports role of p53 mutations in development of head and neck cancer, including SCC arising in IP.Ki67: Immunostaining was positive in all cases except one. Expression rate ranged from 0.03 in papilloma to 0.7 in carcinoma. Statistical analysis using Kruskal Wallis test showed significant difference between 3 groups (p=0.008). Wilcoxon’ s test showed significantly higher values in the group of malignant than in the benign tumors. Conclusions: Our results support the potentially important prognostic role of p53 and Ki67 staining in inverted sinonasal papilloma.
