Sažetak
Background: It is known that young-onset Parkinson’ s disease (PD) is associated with motor fluctuation during chronic treatment. Few studies have investigated the influence of age at onset on quality of life (QoL) in PD patients with contradictory results. Objective: To assess QoL in PD patients. Patients and Methods: We studied ninety-three patients with idiopathic PD. According to age at onset patients were divided in two groups: age at onset before the age of 50 ; young-onset PD and age at onset from 60 years and up ; older onset PD Disease severity and presence of dyskinesias were evaluated using the Unified Parkinson’ s Disease Rating Scale part III and IV (UPDRS). The QoL was rated with the Parkinson’ s Disease 39-item Questionnaire (PDQ-39). Presence of depression was evaluated with Beck Depression Inventory (BDI). Results: Forty four patients had young-onset of PD and 49 patients had onset after 60 years. Patients belonging to the young-onset PD group were younger at time of the study (p<0.01) then older-onset group. Dyskinesias were more frequent and disabling in young-onset PD (p<0.05). According to scores on BDI young-onset PD were more depressed than older-onset patients (p<0.01). QoL was significantly worse in the group of young-onset PD patients (p<0.05).The dimensions of activity of daily living, emotional well-being, and stigma were the most strongly affected. Conclusion: Younger age at PD onset may have significant impact on QoL. This could be partially attributed to more frequent complications and consequently higher level of stigmatization as well as depression in this group of patients.
