Sažetak
Background: The aim of the study was to evaluate changes in stroke therapy according to modern principles, stroke complications and mortality rate in patients who were treated in intensive care unit (ICU) in the last 15 years. Patients and methods: We included data of 2628 (total number9756) patients with severe stroke (NIHSS>15) who died in ICU in the last 15 years - 1122 (44%) men and 1506 (56%) women. Inflammation parameters were evaluated on admission and after 3 days. Blood pressure was measured every 4 hours and mean blood glucose levels were also registered. Four years ago stroke unit was established improving the quality of medical care and early rehabilitation. Data about antiedematous therapy, anticoagulant therapy and antibiotics were also collected. Results: 28-days case fatality in the last 15 years decreased. Mean age increased, especially in women. At admission 64% of patients who died during hospitalization already had increased inflammatory parameters, while at the third day of hospitalization all patients had verified inflammation. There was a decrease in usage of antiedematous therapy, and increase in usage of anticoagulant therapy. During the last l5 years we performed better blood pressure and glucose level controls, and there is a possibility of early rehabilitation in stroke unit.
