Sažetak
Background and purpose: We evaluated the correlation of the melatonin diurnal rhythm and different stroke types and subtypes incidence according to when they happened and risk factors which affect the cardiovascular system such as hypertension, antihypertensive therapy and stenosis of the main blood vessels of the head and neck. Patients and methods: Data of 200 patients admitted to the Neurology Department in the last 5 months were included. All patients underwent CDFI, TCD and CT/MR imaging tests, complete laboratory workup, ECG and chest X-ray. All patients had verified recent ischaemia /haemorrhage of brain parenchyma on CT/MR scan. Results: There were 94 women mean age 74t10 ; 106 men, mean age 68t12. We showed in our study that there is a strong correlation of melatonin diurnal cycle and intracerebral haemorrhage ( I 8°/~ in the morning hours according to blood pressure morning peak), and ischaemic stroke patients (82%) depending on mechanism of ischaemia - in the early morning hours there was a higher incidence of ischaemic stroke patients who had in anamnesis hypertension and irregular antihypertensive therapy (impact of diuretics and beta blockers on melatonin serum concentrations)thromboembolic and small artery disease, while ischaemic stroke was more frequent in the evening hours in patients with symptomatic high grade stenosis and occlusion of the main head and neck blood vessels (according to decrease of the blood pressure and cerebral hypoperfusion). Conclusion: There is a strong correlation between melatonin diurnal cycle and different stroke subtypes according to stroke mechanisms, which should be the target of stroke prevention.
