Sažetak
The purpose of our study was to evaluate the necessity of antimicrobial drug use in a prospective and longitudinal approach at a Department of Medicine at a University Hospital Center Rijeka by using a new scoring system as a quality indicator. Quality indicators that assess antimicrobial drug use evaluate mainly the appropriateness of the choice of an antimicrobial agent. The question that should precede choosing of an antimicrobial is whether the indication for antibiotic therapy has been justified. We have assessed the justification of the indication for antimicrobial treatment by using a scoring system that is based on the presence of signs and symptoms of an infection. We have tested the scoring system developed by Francetic et al. from the University Hospital Centre Rebro (UHC), Zagreb. It seemed appropriate to test this scoring system in the similar institution, in the same country because of the same drug policy and health care services in general. In this audit medical records of all hospitalized patient were reviewed daily during the investigation period. We determined the relative frequency of antimicrobial use for prophylactic and therapeutic indications, defined the relative frequency of antimicrobial prescriptions in different wards, evaluated the relative frequency of prescribing of antimicrobials classified as of restricted release, and identified potentially inappropriate use of antimicrobials based on the explicit scoring system.
