OBJECTIVES: Prostatic adenocarcinoma has been shown to induce a stromal reaction that is associated with changes of stromal cells phenotype as component of tumor progression and spread. The aim of this study was to analyze changes of the stroma in prostatic carcinoma using Mallory trichrome method and to establish their potential significance, especially in diagnostics of minimal carcinoma. MATHERIALS AND METHODS: We analyzed needle core biopsies from 37 patients with minimal prostatic carcinoma, present in one core only. Specimens were fixed in 10% buffered formaldehyde, embedded in paraffin, cut at 4 µ ; m and stained with Mallory trichrome method. By this procedure prostatic stromal smooth muscle cels were stained red and myofibroblasts and collagen fibers were stained green. Proliferation of myofibroblasts and collagen fibers in prostatic stroma were graded semiquantitatively and expressed as 1, for weak, 2, for moderate, and 3, for strong green staining. Normal glands and their stroma within the same core served as a control. In 6 cores tumor tissue was cut out and 31 core biopsies were further analyzed. RESULTS: Patients who’ s biopsies were analyzed were 51-80 years old. 23 tumors (74.2%) were Gleason score 6 or lower and 8 (25.8%) were higher than 6. The most common grade was three. In 30 (96.8%) biopsies one or both grades in Gleason score were three. Tumor tissue was present in 5%-70% of core. In 7 cores (22.6%) stromal reaction within prostatic cancer was graded as 1 and in most cores, 16 (51.6%) it was 2 and in 8 cores three (25.8%). CONCLUSION: Reactive stroma which occurs in prostatic carcinoma showed more intense desmoplastic reaction by Mallory trichrome method in comparison with uninvolved prostatic tissue. Mallory trichrome method could be helpful additional tool in diagnosis of some suspicious cases of minimal carcinoma but studies on larger number of biopsies should be recommended.
