Sažetak
The aim of this work is to present oral manifestations of an aplastic anemia (AA) - a rare syndrome of a hypofunctional bone marrow. In a 26 year old patient nine years ago AA was diagnosed. From that time she was under cyclosporine 100mg a day therapy. In the acute fazes of the disease she was also administrated corticosteroid therapy. Oral medical findings were normal. On the basis of the periodontal findings aggressive periodontitis (AP) was diagnosed. Microbiological PCR tests ( MicroDent i MicroDent plus test ) and periodontitis IL-1 polymorphism risk test were done ( GenoType PST test). The presence of several putative periodontal pathogens were found ( Prevotela intermedia, Porphyromonas gingivalis, Bacteroides forsytus, Treponema denticola, Peptostreptococcus micros, Fusobacterium nucleatum, Campylobacter rectus , Eikinella corodens, i Capnocytophaga sp). IL-1 polymorphism test was negative. Periodontal findings showed severe gingival inflammation, tooth mobility, especially in the upper front and deep periodontal pockets with advanced attachment loss. Periodontal therapy consisted only of initial therapy supplemented with an antibiotic therapy (amoxcylin 500 mg and metronidazol 400 mg three times a day for 8 days). Teeth with poor prognosis were extracted immediately. Following re-evaluation of periodontal conditions after 3 months prosthetic rehabilitation was made utilizing mobile prosthetic appliances. On the basis of oral findings it can be concluded that AA is a disease were an increased risk of onset of aggressive forms of periodontitis occurs. In such patients periodontal therapy must start as soon as possible with mandatory antibiotic supplement.
