Sažetak
Introduction. Alport's syndrome is one of the most common forms of hereditary nephritis which typically follows a progresive bilateral sensorineural hearing impairment and ocular changes. The presence and severity of the symptoms may vary and are closely related to age and sex of the patient. Methods. Due to Alport's syndrome the first twin was surgically treated with renal transplant and the second twin was subdued to a repeated hemodialysis 12 hours a week. After 4 years of transplantation in the first twin and 6 years of hemodialysis in the second twin both were subdued to audiologic examinations. Results. Hearing impairment commonly occurs in Alport's syndrome patients, and is always associated with the symptoms of nephritis, bilateral sensorineural hearing impairment is observed at high frequencies, from 4000 to 8000 Hz. Pure tone audiogram (PTA) was made in both twins at the age of 30. Bilateral sensorineural hearing loss was detected at high-frequency range with a hearing threshold of 55 dB. Conclusion. Renal transplantation in patient with Alport's syndrome in monozygotic twins has proved to have no effect on hearing improvement. Deterioration of hearing after renal transplantation is possibly due to hyperviscosity of the plasma resulting from hyperlipidemia induced by steroid therapy. References. 1.
