Sažetak
The diagnostic value of the transbronchial/transtracheal fine needle aspiration(TBFNA) in the patients with mediastinal nad/or hilar lymphadenopathy has been investigated. Out of 116 patients with mediastinal/hilar lymphadenopathy, the diagnosis of sarcoidosis was established in 88 (75.9%). there were 57 women and 31 men, mean age 43 years, SD+/-13 years. One-hundred-seventy-one TBFNA with cytological 26-gauge needle, from different lymph node stations were performed. Adequate lymph node samples were obtained in 157/171(91.8%)TBFNA and 14/171 (8.2%)samples were inadequate. Cytological findings consistent with sarcoidosis were found in 79/88(89.77%) patients and 133/157(84.71%) materials. In 63/88(71.59%) patients and 104/157(66.24%) materials cytological diagnosis was elements of granuloma, in 16/88 (18.18%) patients and 29/157(18.47%) materials, epitheloid cells among lymphocyte population were found. In the remaining 9/88(10.23%) patients with sarcoidosis and 24/157(15.29%) materials lymph nodes elements or lymph nodes hyperplasia were found. Sensitivity of TBFNA in sarcoidosis presenting as mediastinal/hilar lymphadenopathy was 78.7%, and specificity 92.3%. Positive predictive value for sarcoidosis was 97.8% and negative predictive value 60%. Overall diagnostic accuracy of TBFNA in the diagnosis of sarcoidosis was 86.2% and cytological findings consistent with sarcoidosis were the only morphological diagnosis of sarcoidosis in 63.6 %of patients.
