The role of cytolytic T cells both CD8+ and CD4+ subsets, in the pathogenesis of tissue damage in multiple sclerosis MS is still far from being completely understood. It is supposed that T cells, after activation in the periphery cross the blood-brain barrier and enter the central nervous system parenchyma where they recognise myelin autoantigens. Changes in the percentages, absolute cell counts and functional activity of natural killer cells were found as well. Both subsets of cytolytic cells, T lymphocites and NK cells, contain in their granules cytolytic protein perforin, which is a mediator in cell-mediated cytotoxicity reactions. In the present study we investigated the expression of P in subpopulations of PBL in MS patients, in active phase of disease, treated with corticosteriod, Methylprednisolone 0.5 g for day, for a period of 5 days. Samples of PB were taken before therapy was introduced, day 0, and at 1, 5 and 14 days thereafter. Perforin and cells surface antigens were detected simultaneosly by flow-cytometry. The results obtained show that corticosteroids could have inportant role in the regulation of P expression. Downregulation of P expression in PBL was found on the 5th day of treatment with corticosteroids. A significant decrease of the percentage of P+ cells was found for total P+ cells and their T-cells subsets. The decrease in P expression was present in the CD16+ P+ subsets of NK cells as well.
