Sažetak
Erectile dysfunction (ED) as incapability to attain, sustain and repeatedly acquire penile erection adequate to perform sexual intercourse is a frequent complication in male spinal cord injuries (SCI) patients. Five male patients aged 24 to 31 years with lumbar SCI were included in study, all admitted to the rehabilitation treatment during the y. 2000 in Special Rehabilitation Hospital in Varazdinske Toplice, Croatia. Beside standard physical, laboratory and neurological examination all patients underwent psychological testing and completed International Index of Erectile Function (IIEF) form. The treatment consisted of Sildenafil given once a day in standard dose of 25 or 50 mg. Treatment lasted for three months of stationary rehabilitation, with three months of follow up. Three patients were ASIA-A, and two were ASIA-B grade paraplegia. All patients were married before injury and two of them had children. By IIEF test scores 3 patients had severe, 1 had moderate and 1 had light form of ED. Therapy was successful in all 5 patients. Initial dose was 25 mg, and had to be increased to 50 mg per day in 4 of 5 patients. Only one patient achieved sufficient erection with 25 mg of Sildenafil daily. The positive effect was sustained during the follow up period in all 5 patients. Sildenafil (Viagra) appears to be treatment of choice in cases of ED caused by SCI. Beside small number of patients, we consider that Sildenafil was used in right indication and appropriate dose. High price of medication was a main limiting factor in wider use of Sildenafil for ED in SCI patients in Croatia.
