Sažetak
Background: In the last decade in Croatia the number of SCI patients has increased. Consequently the number of hospital beds has increased as well. Since we have made our Specific Croatian Model of research and treatment based on the experience from the other parts of the world. Objectives: Authors present organisation and modalities of treatment of spinal cord injuries in developing countriy. Croatia is today a rapidly developing country with four and a half million inhabitants. Recent sudden increase of incidence of spinal cord injuries has intensified a scientific researches and technical development in treatment and rehabilitation of such patients. Methods and subjects: Authors have observed patients with spinal cord injury (SCI) in Croatia throughout 10 year period. Patients were treated in the University Clinic for Traumatology in Zagreb, and continued their rehabilitation in the Special Rehabilitation Hospital in Varazdinske Toplice. Continuous follow-up lasted from 6 to 10 months after the injury. All applicable statistic methods for data processing were used. In the earlier period specific modes of treatment of spinal injury in given circumstances have been described and results thoroughly analysed. Findings: Croatia today is a country with approximately 4, 5 million inhabitants. 1356 patients were treated in 10-year period from 1991 to 2001. Age of patients varied between 2 and 91 years. 68 patients were lost in the study for different reasons. During the first five years the main cause of injury were war injuries, while in the second five year the cause of injuries followed pattern known in other parts of the world, traffic trauma being the leading cause of injury. Only two cases of spinal cord injury could be connected with sports. Average time spent in surgical units was 61 and in rehabilitation units 141 days respectively, with diminishing trends in peacetime period. Conclusion: Spinal cord injuries are the most serious injuries connected with function of locomotor system. Their consequence is always some degree of disability depending on the level and intensity of initial lesion. In developing countries as Croatia, although general principles of treatment follow those accepted in other parts of the world, the reintegration process is delayed and attenuated by other social problems. Possible courses of further development are discussed.
