Sažetak
Mechanical hyperventilation is widely used in patients with severe head injuries during last 20 years. Recently so called optimized hyperventilation was proposed for neurotraumatized patients. The purpose of study was to find if intermittent measurements of SvjO2 in severe head injuried patients will influence the setting of ventilator. 31 patients with severe head injury were included in the study. All the patients were mechanically hyperventilated to PaCO2 between 3,5 and 4,0 kPa. For SvjO2 measurement venous catheter was placed by percutaneous retrograde canulation of internal jugular vein. In 13 of 31 patients SvjO2 55 mm Hg was found in 24 measurements. After lowering minute volume of mechanical ventilation, in 12 of 13 patients satisfactory values of SvjO2 were obtained.
This results suggest that even with intermittent measurement of SvjO2 better setting of mechanical ventilation for neurotraumatized patients can be achieved.
