Sažetak
The Unit for Clinical Toxicological Chemistry of the Institute has over 20 years' experience of participating in international quality assurance programs concerning analysis of biomarkers of metal exposure and effect in humans. It was started with participation in the external quality control program for blood lead, organized by the Commission of the European Communities (1978-1980), and for blood lead and cadmium, organized by the World Health Oganization within the framework of the WHO/UNEP project "Biological Monitoring of Human Exposure to Lead and Cadmium" (1980-1982). It was continued by regular participation in the U.S. Proficiency Testing Program for protoporphyrin in blood, which is a characteristic biomarker of lead exposure and/or effect, organized by the Centers for Disease Control (1983-), the U.K. National External Quality Assessment Scheme for blood lead and cadmium (1983-), and the U.K. Trace Elements External Quality Assessment Scheme for serum copper, zinc and selenium (1995-). A relative advantage of the former two of the above five programs is in the fact that, apart from the unknown sample, two internal quality control samples with specified value of the analyte were distributed in each round, while a target value of unknown samples was precisely defined by selected competent laboratories. The latter three programs are based on distribution of only unknown samples, each round in two week intervals, while a target value is defined on the basis of mean value of results reported from the participating laboratories. As the analytical quality of all the participating laboratories may not be sufficiently high, resulting in wide scatter of the reported values, a relative disadvantage of such programs is in poorer reliability of a target value of the analyte. In conclusion, regular participation in international quality assurance programs is considered essential for the accuracy of metal analyses in human biological specimens, particularly because subtle changes in metal biomarkers, even those within a range of so-called "normal values", can be associated with significant alterations in health effects. This is especially important with regard to the increasing concern about oxidative stress-mediated health effects of interactions between toxic and/or essential metals in human general population. Therefore accurate analyses of biomarkers of several relevant metals are necessary for evaluation of health significance.
