Transrectal prostatic fine-needle aspiration cytology is a widely used diagnostic method due to its high degree of accuracy and absence of serious complications. The purpose of this investigation was to compare morphologic findings with DNA ploidy and proliferative activity in various prostatic lesions and to find out if it could be used to improve cytologic diagnosis. Method: We analysed DNA ploidy and proliferation in 101 patients with cytologic diagnosis: glandular hyperplasia, atypical glandular hyperplasia, cancer and normal prostate, by flow cytometry. Results: Aneuploidy was found in 16 (15.8%) lesions: 2 (11%) of 18 normal prostate, none of 30 glandular hyperplasia, 1 (3%) of 34 atypical glandular hyperplasia and 13 (68%) of 19 cancers. High proliferative activity was found in 49 (48.5%) prostatic lesions: 4 (22%) normal prostates, 6 (20%) glandular hyperplasias, 22 (65%) atypical glandular hyperplasias and 17 (90%) prostatic cancers. Aneuploidy correlated positively with poorly differentiated tumour and high proliferative activity correlated positively with atypical glandular hyperplasia and cancer (Fisher's exact probability test p <0.05). Discussion: Flow cytometric analysis of DNA ploidy and proliferative activity of fine needle aspirates in prostatic lesions appears to be a valuable supplement of morphologic criteria, especially for prognosis of prostatic cancer.
