Sažetak
We report on a demented patient with extrapyramidal, cerebellar and lower motor neuron involvement. He was 52 at onset of the disease and died two years later. The presenting neurological pattern was: dysphasia, agnosia and slowness of thought and movement. A year later diffuse fasciculations and marked muscular atrophy were observed in all extremities and tongue together with dysarthria, hypodiadochokinesia, Parkinson-like features and paresis of all extremities. CT scan of the brain revealed central and cortical atrophic changes with marked enlargement of the frontal and Sylvian fissures bilaterally. Gross examination revealed atrophic changes in the left parietal and upper temporal cortex ; microscopical examination showed degeneration with achromasic neurons and chromatolytic-type changes in the affected cortical regions, pigmented and motor nuclei of the brainstem, dentate nucleus and spinal cord motor neurons.
