Sažetak
Schizophrenia is a chronic psychotic mental disorder with a progressive damaging course. Therefore, an early successful treatment is of greatest importance, which is also useful for the cost/benefit analysis, because the treatment of schizophrenia lasts for years, and occasionaly for life. This case presented points to good therapeutic effects of olanzapine treatment on outpatient basis, without undesirable therapeutic effects in a young patient with the picture of first acute psychosis. Before the induction of olanzapin in therapy, the patient's condition was assessed using three scales: PANSS, CGI severity, Simpson-Angus rating scale for EPS. The patient was folowed up every two weeks for two months. The results contained were consistent with previous studies. Bsed on the case presented, it is concluded that an atypical antipsychotic is therapy of choice for the first psychotic episode in a young patient, because it reduces both positive and negative psychotic symptoms, is no associated with extrapyramidal side effects, is simple to use, contributes to patient compliance, allows for early reintegration, and avoids the need of hospital treatment. Also, the historical pessimistic view on the presumed static nature of cerebral dysfunction in schizophrenia should be modified, emphasizing the possibility of a more positive prognosis thanbefore.
