Sažetak
The aim of this paper was the determination of the level of plasma polymorphonuclear (PMN) elastase, as a contribution to the evaluation of acute pancreatic disease. In the group of patients (N=42), levels of plasma PMN elastase, catalytic activities of alpha amylase in serum and urine, and lipase, were studied in the acute phase of the disease. For the determination of PMN elastase level, the IMAC technique of MERCK was used, and other enzyme activity analyses were done using the tests of HERBOS. The study results indicate a significant increase in PMN elastase level in the patient group (mean value=94.4 +/-36.3 ľg/L) compared to the control group (N=28, mean value=21.6 +/-7.9 ľg/L). Also, a significant increase in the catalytic activities of alpha amylase (serum and urin) and lipase ensued. To assess the differential diagnostic value of individual parameters , and with the aim of better diagnosis of acute pancreatitis, ROC curves were done, which showed that PMN elastase levels and catalytic activities of examined enzymes have the greatest value for discrimination between patients and controls. In view of these results, it can be concluded that the determination of plasma PMN elastase level significantly contributes to better differential diagnosis of acute pancreatic disease.
