Sažetak
Brown naphthalene has been used for 15 years in the treatment of psoriasis at Special Hospital for Medical Rehabilitation in Ivanic Grad near Zagreb, Croatia. Naphthalene is obtained from naphtha, a complex mixture of various compounds, mostly hydrocarbons. Naphthalene AA-final is a new naphthalene preparation enriched with sterans, constituents believed to represent active naphthalene components, while the content of polyaromatics has been substantially reduced. Experimental model studies revealed no genotoxicity. The preparation is available in the form of oil. To assess the effects of Naphthalene AA-final on proliferative activity in the epidermis, epidermal and dermal CD4 and CD8 lymphocyte count, apoptotic cell count in the epidermis, and antiangiogenic activity. Ten patients (four female and six male), mean age 49.5 (range 40-63) years, were included in this pilot study. Mean PASI score at the beginning of therapy was 22.4. Naphthalene AA-final was applied over involved skin areas once daily. After 30 min, the patients removed residual preparation from the skin and took a shower. Biopsy specimen was obtained from target lesions before and after three weeks of treatment. The number of proliferating keratinocytes was determined immunohistochemically by use of Ki-67 antibodies as number of positive cells/mm epidermis length, CD4 and CD8 cell count/mm epidermis length and /mm2 dermis. The number of neovascular formations/mm2 dermis was measured by use of CD31 antibodies. Cell count expressing apoptotic oncogenes was determined by use of anti bcl-2 antibodies. The mean PASI score at the end of therapy was 7.85. The use Naphthalene AA-final resulted in a decrease in the following parameters: Ki-67, p=0.022 ; CD4 (epidermis) p=0.009 ; CD4 (dermis) p=0.05 ; CD8 (epidermis) p=0.002 ; CD8 (dermis) p=0.005 ; and CD 31 p= 0.05. The bcl-2 positive cell count showed no significant difference before and after therapy (p=0.053). The observed effects of naphthalene therapy on the proliferative activity in the epidermis, number of immunocompetent cells in the dermis and epidermis, along with its antiangiogenic action suggest it to be a local therapeutic option for mild to moderate forms of psoriasis.
